Dan Ganz, MD (Dan Ganz MD PLLC) and Curvy Plastic Surgery, the Miami practice where Dr. Ganz sees patients.
How
By telephone call, SMS/MMS text message, and email, at the phone number and email address you provide on the consultation request form.
What we may contact you about
We may contact you to respond to your consultation request, including arranging, confirming, rescheduling, or following up on the consultation you requested. We may also reach out to ask about your experience and any feedback you may wish to share.
We will not send you promotional or marketing messages under this consent.
Further communications
Any communications beyond the scope described above, including appointment reminders for scheduled procedures, care instructions, events, newsletters, or promotional messages, are sent under the practice's own terms and require your separate consent given to the practice directly. Submitting this form does not provide that consent. Consent may also be collected in writing at the practice.
Message frequency
Varies based on your request and scheduling. Message and data rates may apply. Carriers are not liable for delayed or undelivered messages.
Privacy of text and email
Text and email are convenient but not fully private. Messages can be read by anyone with access to your phone or email account, may stay visible on a lock screen or a shared or synced device, and pass through the carriers and service providers that deliver them. We keep these messages brief and limit what we include, but a message from us may reveal that you have contacted a cosmetic surgery practice.
If you would prefer a more private method such as a phone call, a letter, or a message through a secure patient portal, tell us and we will use it instead. You may also ask us to contact you only at a specific number, address, or time. You can change your preference at any time.
How to opt out
Reply STOP to any text message to stop receiving text messages. You may ask us to stop calling or emailing at any time by replying to any email or telling us by phone. Opting out will not affect your ability to schedule or receive care.
Consent is optional
Your consent is not a condition of receiving care or purchasing any product or service. You may request a consultation by calling the office directly instead of using this form.
How we handle your information
We do not sell your personal or health information, and we do not share it with third parties for their own marketing purposes. We share it only with Dr. Ganz and his practice at Curvy Plastic Surgery for the purpose of scheduling and treating you, and requesting feedback to improve others' experiences.
Changes to these terms
We may update these terms. The version in effect at the time you submitted your request governs that submission. Material changes will be reflected here with a revised "Last updated" date.
